Please complete and submit this invoice after the 10th day of classes of each academic term. Commissions will be paid only to registered companies (not to individuals) and only after receipt of this invoice. Invoices must be received within 90 days of the start of each term. If commission is not claimed within the 90 period the commission will be forfeited. Sheridan will issue commission payments to the company and address listed on this invoice. All cheques will be issued in the company’s name.
