Had an Accident or Incident
If you have experienced a workplace incident or accident, you must notify your supervisor or manager immediately. An Incident Report must be completed and submitted to OHS Services within 24 hours.
Note: Students who suffer an injury at Sheridan should contact Security or the Health Centre. Visitors and Contractors are to report any incidents to Security. Contact information is available below.

An Incident Report must be completed if an employee suffers any of the below incidents:
• Workplace Accident – Employee suffers an injury or disablement while at work. These include ergonomic injuries such as sprains/strains from the workplace.
• Occupational Illness – Employee suffers an illness from exposure to a physical, chemical or biological agent while at work.
• Near Miss Incident – An incident that did not injury an employee however had the potential to cause injury. This may include an incident which caused property damage.
• Workplace Violence/Harassment Incident – An employee who has suffered an injury or illness due to workplace violence and/or harassment. To review the college’s policies in these areas, please click on the policy below.

Who Completes the Incident Report?
The supervisor or manager is required to complete and sign-off on the incident report. A completed report must be forwarded to OHS Services within 24 hours.
Information collected on the Incident Report
The Incident Report is designed to capture as much information about an Incident. These include: Details of the Affected Individual, Incident Details, Root Causes Analysis and Prevention Plan. When completing an Incident report, try and capture as much information as possible. A properly completed Incident report will ensure that an effective prevention plan is implemented to ensure a similar incident does not occur.
Details provided on the Incident Report will be used to ensure a proper prevention plan has been put in place. It may also determine if any training programs should be put into place in the future.
