When is an appointment needed?

Appointments are required to see a physician. Health services by Registered Nurses are offered on a first-come first-served walk-in basis (for non-emergencies) during all Health Services operating hours.

How to book an appointment:

Appointments can be booked at the Health Centre locations after consultation with nurses. 
Please let reception know if you require a longer appointment i.e. for a physical, Pap examination or talk time. It is recommended that you book appointments with the same doctor to ensure continuity of care through someone that can become familiar with your health needs. 

What do you need for your appointment:

A Campus Health Services Registration Form, must be completed prior to a first appointment with a physician. 
An Ontario Health Insurance Plan Card (OHIP), other Canadian provincial Health Card or Guard Me Insurance Card must be presented at each appointment.
Please be advised to also bring your OneCard to every appointment. 

Missed Appointments:

Please be advised that you are required to give notice for cancellation of an appointment with a doctor. A message can be left on the Health Centre’s voicemail or students may simply speak to one of the nurses at the Health Centre.

Failure to notify the Health Centre could result in a $50 charge for a first missed appointment and $50 for any additional missed appointments without providing appropriate notice.

Fee charges will be applied to your Sheridan account

Doctor's Notes:

Doctor notes will not be provided unless you were treated in the Health Centre on the day when you were sick.
